If you were evaluated by Dr. Fallis or Dr. Womack per a court order, your lawyer, or another contracting entity, your evaluation report and associated documents (not including test material protected by copyright) do not belong to the psychologists but may be requested through the court, lawyer, or other contracting entity. In other words, the evaluation report and associated documents are third-party documents and are in the psychologists custody but may not be provided to others without appropriate permission or a court order.

If you received services from Dr. Fallis or Dr. Womack which were not covered by a court order, attorney-client privilege, or contracting entity restrictions, you may request healthcare records from Dr. Fallis or Dr. Womack by submitting a written request (email or letter) directly to Dr. Fallis or Dr. Womack, using some form of identification to verify your identity. Please specify what records are needed and a timeframe. Note that reasonable costs for copying and mailing may apply, authorized under Texas law. If the provider or providers are deceased, please contact the executor of the estate or the designated custodian.

Regulatory complaints are handled by the Texas Behavioral Health Executive Council (BHEC), while consumer-protection complaints are filed with the Texas Attorney General.
